Outcome’s Portal offers an integrated Closed Loop Referral module, which allows clinicians and care takers to identify Community Based Organizations (CBOs) and programs the patient may be eligible for.
Patient-Centric Care Coordination
Within the Patient Summary page, users can initiate a search using a variety of criteria:
- Patient location
- Patient age
- Domains: Housing Stability, Primary Care and Prevention, Health Management, Nutrition and Food Security, Financial Wellness and Benefits, Activities of Daily Living, Social and Community Connection, Legal and Criminal Justice, Safety and Disaster, Utility and Technology, Transportation, Education and Human Development, Personal Care and Household Goods, Employment Development and more
- Food Insecurity Level: Low, Medium and High
- Housing Status: Temporary Crisis, Low to Moderate Barriers, High Barriers, Extreme Barriers / Long Term Crisis
Easily Build Custom Forms and Assessments
While delivery care, Clinicians and other members of the Care Team can initiate referrals within the Patient’s Chart.
